Transportation Student Information Form
Help us ensure we know how every student should be dismissed after school. We are using an online form this year to collect information from parents on information like car riders, bus numbers, and other dismissal information.
We ask parents and guardians to complete the TNS Transportation Student Information Form (link). You can also access the form by scanning the QR code below. Complete one form per student. Please do not put multiple children on one form. This will ensure we have as much information as possible on how the students are being dismissed on the first day of school and following on the next school days. This information will only be used for the school and not made public.
It is highly recommended that your child come to school on the first day the same way they will go home. If the child rides the bus in the morning, they'll get to meet the bus driver and other students that ride their bus. This enables the child to have additional points of reference to help them find their bus in the afternoon other than just relying on their bus number.

- If you need to make a change in transportation for dismissal just for that day for your child throughout the school year, you will need to complete that request using the TNS Attendance & Dismissal Form.
